Together, we’re changing lives.

The Street Outreach initiative brings health care, vital resources, and personalized guidance to vulnerable neighbors. Led by a registered nurse and community health workers, this team reaches neighbors where they are by helping with:

  • Assessing needs and navigating resources (e.g., food assistance, transportation, health insurance enrollment, debt relief, shelter). Scheduling medical, dental, and mental health/counseling appointments.  
  • Administering over-the-counter medications and safely storing prescription medications, preventing theft, misuse, and misplacement.
  • Performing health screenings and vaccinations. Partnering to complete applications for safety-net programs, housing, employment, and more.
  • Providing education and lifesaving supplies (e.g., blankets, socks, boots, food). 

This no-barriers approach to caring for our community members is the driving force behind 
the program. Philanthropy makes it possible.

Right Care

Suffering from debilitating central tremors, John needed the outreach team’s help to coordinate a medical procedure for hand stabilization. 
Now, he is housed, working, and thriving.

Right Place

  • Mistrust of the health care system is a pervasive issue that directly affects access and outcomes.
  • Delayed health care is more costly and less effective.
  • Receiving help in places of familiarity (e.g., community centers, churches, shelters) can reduce stress and promote trust.

Facing a life-threatening medical emergency but lacking health insurance, Abdulsalam hesitated to seek care. Susmita helped him enroll in a health insurance plan so he could get the care he needed without financial stress.

Right Time

  • Timely intervention can prevent crises from occurring, decreasing the rates of hospitalization and avoidable complications.    
  • Contacting patients after hospital discharge helps prevent readmission and identify complications early.

With the outreach team’s support and care coordination, Eddie entered recovery. He is now more than 120 days sober.
